2026 -27 Children & Youth Formation Registration

Please use the following form to register children (preK-6th grade) & youth (6th-12th) in your family. Add each child's name, birth date, allergies, medical needs, grade, and anything else you would like to share.

You can register up to 3 members of your family under a single registration. When registering more that 3 children, please complete a second registration form.

Regarding photographs and videos of my child taken at any St. Anne’s Episcopal Church events, I give permission to do the following for nonprofit use and without charge: use at the discretion of St. Anne’s Episcopal Church, display at a service or event or be used in a multimedia presentation, reprint and distribute for any St. Anne’s non-profit publication to accompany when used (for example, in the enews, brochures, emails, etc.), to include quotes, display photographs or videos clips on the St. Anne’s Episcopal Church website, on St. Anne’s social media pages. Therefore, I agree to indemnify and hold harmless the church, its trustees, rector, associate rectors, vestry, members, and designees from any claims arising out of the use of my child’s photographs and videos.

Opportunities to Support St. Anne's Mission to Children & Youth

In addition to our talented Sunday School teachers, we need a second adult volunteer in the room each Sunday. We are asking each family with children in Sunday School to volunteer in a Sunday School room twice during the program year.

I'd be delighted to help out! This would be a difficult lift for our family. Please contact me. I'm only registering for youth ministry

We also need volunteers for our youth ministries. Are you interested in forming relationships with our youth (6th-12th grade) by being present at youth events once per month? We have programs every week on Sunday afternoons.
